* To qualify as a Trainee, applicants must submit a letter from your department administration office or Divisional Head as proof of full time enrolment in a specialty-training program in the current academic year. All letters must be sent to the LUPUS 2010 Congress Secretariat - attn: Bryanna Thiel by email (lupus2010reg@advance-group.com) or fax (604-685-3521) upon registration.

** To qualify as an Allied Health Care Professional applicants must submit proof of employment or certification as a physiotherapist, occupational therapists, social worker, psychologists, research assistants and registered nurses. A copy of a letter from your department administration office or similar must be sent to the LUPUS 2010 Congress Secretariat.

***An Accompanying Person is a Participant’s spouse, partner, child or other personal relation who does not have a business or scientific interest in the Congress. The Accompanying Person’s program includes Exhibition Access, the Welcome Reception and the Potlatch Reception.

Payments
All payments must be made in US Dollars.

Full payment of registration fees is required to attend the LUPUS 2010 Congress. Upon receipt of your registration form and payment, the Congress Secretariat will send an email confirmation to the Participant. Please ensure that you keep this confirmation for your records. It is also recommended that you bring a copy with you to the Congress.

Payment Methods
• Visa, MasterCard or American Express are accepted credit cards (the charge will be posted on your credit card statement as Advance Group)
• Company Cheque (Payable to: LUPUS 2010 c/o Advance Group)
• Bank wire transfer (an additional $25 USD fee is applicable)

Registration Cancellation Policy
• Please submit your cancellation or change request in writing to the email address below.
• All cancellations and modifications must be submitted in writing to the LUPUS 2010 Congress Secretariat
• An alternate attendee name may be substituted if you are not able to attend the Congress.
• Refunds for cancellation will be granted less a $100 USD cancellation fee and $50 USD administration fee per registrant provided the LUPUS 2010 Congress Secretariat is notified in writing by March 15, 2010.
• Cancellations received from March 16, 2010 up to and including May 17, 2010 will result in a 50% refund of the Congress fees less the cancellation and administration fee.
• Cancellations received starting May 18, 2010 are 100% non-refundable.
• All changes are subject to a $50 USD administrative fee.
• Credit Cards that need to be reprocessed are subject to a $75 USD administration fee.
• All refunds will be processed and issued within 30 days after the Congress.
